Fixed Web Book Site Export Bug

As a result of changes made in Version 18.4.0, the Web Book Export as Site function was deleting all output image files, but only replacing them if the corresponding note changed. This has been corrected, so that all image files will be rewritten.

Eliminated Tahoe Menu Item Images

There have been several recent blog posts decrying macOS Tahoe’s rather liberal and indiscriminate scattering of small image icons across an app’s menus. See this Rogue Amoeba blog post for a summary. This change removes all such visual flotsam, using this code from Brent Simmons.
